By a simple petition in writing bearing the following information:
- Full names, occupation and place of residence of the complainant;
- The name of the accused official or service within the Regional Assembly or Council Administrations;
- A clear statement of the facts that justify the complaint; The argument put forward;
- Where possible, any exhibits or evidence submitted to back the petition;
- Direct oral complaints may be admitted. In this case, minutes shall be taken down and signed by the complainant.
